Vista: Getting an Account

To apply for access to Vista, please complete the Vista Access Request Form. If you haven't already, also complete UCSC's Access to Information Statement (both forms are below) and send them to:

• Download the UCSC VISTA Access Request Form in Microsoft Word format
• University Access to Information Statement (required, if not already on file)

Once your account has been established, someone from the Vista Team will contact you with your User Name and Password.
